Hi, I have a Problem with the Activation of Photoshop CS 3 Extended (student edition).

In the Covering is a piece of paper with a link:

Now, I have a problem with step 3.  "Fill out the online Student Order Fulfillment form". I can't find this. Does anyone know, where I can find the student order fulfillment form?

You are supposed to get the form when buying the product (according to some sites I found with Google).
Where did you get it [Photoshop]?

I bought it in the PC-Shop of my village. I think, I write a question to the Adobe Support .
